Survivorship Care Plans

The Institute of Medicine recommends that all patients who complete their cancer treatment are provided with a survivorship care plan that includes information about their treatment, plans for follow-up, possible late effects, resources, and care coordination. Several organizations have developed programs to create survivorship care plans for patients.   

LIVESTRONG Survivorship Care Plan
Developed in tandem with the University of Pennsylvania, this site allows a clinician or patient to create an individualized survivorship care plan. The summary provides information about possible late effects and follow-up care.

Free software for professionals to create custom survivorship care plans for patients. These plans outline the recommended testing for surveillance, focusing on prevention, and screening for other cancers.
