This organization includes resources focusing on exercise and rehabilitation for cancer survivors.  Moving Through Cancer aims to assure that all people living with and beyond cancer are assessed, advised, referred to and engaged in appropriate exercise and rehabilitation programming as a standard of care.

Survivorship care plans

The Institute of Medicine recommends that all patients who complete their cancer treatment are provided with a survivorship care plan that includes information about their treatment, plans for follow-up, possible late effects, resources, and care coordination.

Several organizations have developed programs to create survivorship care plans for patients. (See Key Resources)
